No ungraded runs. Outcome (useful / partial / failed) + failure mode (grounding · instruction-following · format · tool use · hallucination). The grade is the training signal.
Most failures are harness failures. Iterate prompt, template, context, tools before blaming the model; re-run; log the delta. Templates are versioned with tracked success rates.
Graded runs feed the trust ledger → watch → queue → auto → bigger seats (voter → worker → advisor). The graded corpus compounds → fine-tune/distill → the model itself moves toward frontier.
